Clinical trial inspection preparation: who can retrieve the source record?

A clinical trial team should know where records are held and who is authorized to retrieve them. A confident answer about location is less useful than a successful, appropriately controlled retrieval.

Map the record before the request

For a selected study process, identify the site, sponsor, and vendor responsibilities, systems used, retained versions, and access arrangements. Separate the person who can retrieve a file from the person qualified to explain the work it documents.

Practice with authorized training material

Use a permitted example to trace original evidence, changes, review, and retention. Do not expose unrelated participant information merely to demonstrate a process. Record missing access, unclear ownership, or inconsistent version references as follow-up questions rather than manufacture a replacement explanation.

Clarify the inspection context

FDA’s December 2025 guidance on processes and practices applicable to bioresearch monitoring inspections provides relevant context. Read its scope alongside the requirements applicable to the actual inspected party and activity. Clinical investigators conducting studies should not be confused with FDA field investigators conducting agency inspections.

Make the handoff repeatable

A practical internal retrieval map can identify the record, custodian, authorized retriever, review contact, and unresolved access issue. This suggested tool does not replace study documentation or an applicable regulatory requirement.
