Why did we receive a Form 483 after passing an ISO audit?
A successful ISO audit and an FDA Form 483 can coexist. The first step is to compare what each assessment actually covered rather than assume one invalidates the other.
Start with the meaning of the documents
A Form 483 records observations made during an FDA inspection; it is not a final agency compliance determination. An ISO audit has its own standard, certification scope, sampling, timing, and evidence. Compare those specific facts in your case.
For devices, read the current QMSR context
The device QMSR became effective February 2, 2026, incorporating ISO 13485:2016 by reference with FDA-specific requirements. FDA’s QMSR FAQ explains the role and limits of ISO certification in relation to FDA inspections. Do not assume that holding a certificate substitutes for meeting applicable FDA requirements or prevents an observation.
Use the difference as a review question
- Were the same sites, products, activities, and periods assessed?
- Was the issue present but outside the earlier sample or scope?
- Did the operation, personnel, configuration, or evidence change?
- Were FDA-specific obligations assessed as well as the applicable ISO standard?
- What does the original evidence show, and what remains uncertain?
Respond to the actual issue
Investigate the observation on its evidence. Do not use a certificate as a rebuttal without addressing the point raised, and do not conclude that the earlier auditor necessarily acted improperly. A comparison can identify gaps in internal assurance while preserving a factual response.
